“What medications do you take at home?”
This is one of many questions that is asked of us when visiting an emergency room, urgent care center, or even a new doctor’s office; yet many individuals may not be able to answer it. Being able to accurately answer this question, along with the follow-up questions regarding strengths and how often it is taken, can be vital for proper care and prevention of medical errors. A study published in the Journal of General Internal Medicine looked at 651 patients being admitted into the hospital and found that 35.9% experienced medication errors. Of these errors, 85% originated in medication histories that were inaccurate or missing.
INDIANA – In celebration of one year in their new location, Gatti Pharmacy will host a week-long event with daily giveaways and more from Monday, November 20th to Saturday, November 25th.
Now located at 1024 Philadelphia Street, Gatti Pharmacy will kick-off the official holiday season with a commemoration of their 2016 relocation.
Last month, as with every October, we celebrated American Pharmacists Month, which is a great reminder to have a medication review with one of our pharmacists. A medication review, or "check-up," can reduce your risk of drug interactions and side effects and help determine if you're taking anything that is unnecessary. Another major benefit of a medication check is that a pharmacist can help to determine if there are less expensive medications to treat your health concerns. As the medication experts, pharmacists are trained to carefully and thoroughly evaluate your medication-related issues. Then, we act as members of the healthcare team by communication with our other providers (and you!) to solve your problems.
